TO QUALIFY:

Per month, simply menu the required number of Kellogg’s® and/or Keebler® products on your breakfast AND mainline
lunch menus. A copy of 1 menu is required to qualify each semester.

INSTRUCTIONS:

  1. A. Fill in the requested contact information, and upload copies of your menu with Kellogg’s® or Keebler® products
    or
    B. Fill in the requested contact information, print this form, attach copies of your menu with Kellogg’s®
    or Keebler® products and mail to the address below
  2. If the Kellogg’s® or Keebler® brand is specified for every menu placement, simply mail or upload a copy of
    this form along with a copy of your menu for verification. If the menu does not list the Kellogg’s® or Keebler® brand specifically, a copy of your distributor
    velocity report or invoices must be submitted along with a copy of your menu and this form for verification.
  3. Determine the tools you qualify for based on menu placements.
  4. Select the participation-building tools for your qualifying level. When selecting your tools, it is an a la carte option.
    You can select a variety of tools or the same tool multiple times.

RULES:

  • Offer limited to foodservice operators in K-12 schools only.
  • Submissions must be made by the school district. Individual school submissions will not be accepted. The tools earned will be shipped to the location designated by the school district.
  • Program time period is August 1, 2006 – June 30, 2007. Maximum one redemption per semester.
  • Tony the Tiger™ visit subject to timing and availability. One Tony the Tiger™ visit per semester for the Gold level. Redemptions received after April 15, 2007 are subject to fulfillment the following school semester.
  • Not redeemable in conjunction with other Kellogg’s® Food Away From Home offers on the same products.
  • Please allow up to 4-6 weeks for delivery of materials.
  • Offer good while supplies last.
  • Cereal qualifies for only four (4) menu placements including breakfast and mainline lunch.
  • A minimum of one menu is required to qualify each semester. A distributor velocity report must be submitted along with this form for verification if your menu is not branded.
  • Gold level minimum menu placements per semester: 24 for breakfast and 18 for mainline lunch.
  • Silver level minimum menu placements per semester: 12 for breakfast and 9 for mainline lunch.
